Request For Investigation Form 
Any person(s) may request that the Commission undertake an investigation by submitting a Request For Investigation Form setting forth an allegation or allegations that constitute potential violations of the Campaign Reporting Act‚ the Lobbying Act‚ the Personal Financial Disclosure Act‚ or the Uniform Recall Election Law. The information contained in the Request For Investigation should be as specific as possible‚ identifying the name of the subject candidate and/or entity‚ the election and/or calendar year‚ and as much detail and support for the allegation(s) as is known.
Your Request For Investigation Form can be submitted by completing the fillable form and uploading it along with any documents relevant to your allegation(s) or by sending your completed Request For Investigation Form and any attachments to the attention of the Review and Investigation Section. If sending via U.S. Mail‚ send to NJ Election Law Enforcement Commission‚ P.O. Box 185‚ Trenton‚ NJ 08625-0185. When using commercial or any other delivery service‚ send to NJ Election Law Enforcement Commission‚ 25 South Stockton Street‚ 5th Floor‚ Trenton‚ New Jersey 08608. All Request For Investigation Forms submitted must provide the name and mailing address of the person(s) submitting the request‚ as well as a day and evening phone number. The Commission does not consider any Requests For Investigation that are submitted anonymously.
Please note the Commission staff will not provide any information regarding receipt of your Request For Investigation or confirm or deny any investigative activity. This procedure is in place in an effort to maintain confidentiality of the requestor(s).

At the conclusion of an investigation‚ the Commission will determine to take no further action‚ issue a letter of instruction‚ or undertake formal complaint action for an alleged violation and seek imposition of civil penalties.
